Directions:

1. Fill in the address portion of the pre-paid UPS – GROUND shipping label on the outside of the Mephisto Shipping Bag.

2. Make sure you printed two copies of this form and complete both copies, keeping one copy for your records.

3. Place shoes or sandals in the Mephisto Shipping Bag. (Please, send only one pair per bag)

4. Enclose your check or money order, made payable to Mephisto USA, Inc. (SORRY, NO CREDIT CARDS).

5. Remove adhesive strip on the bag closure flap and fold flat to seal.

6. Mephisto will pay the shipping charges for your package if shipped from any authorized UPS shipping location or any regular USP pickup stop. To find the nearest   authorized USP shipping location call 1-800-742-5877. For a UPS fee of $10.00 a special pickup may be made by calling UPS at 1-800-377-4877, ext. 2812 and identify yourself as a

ARS-Mephisto Shipper and request a pickup.

7. Recraft orders without the required information may be returned to you unprocessed. Be sure to include your daytime phone number and a check or money order.

8. ***Very Important*** Please write the UPS – GROUND Tracking number in the provided space at the top right hand corner of this form. This is the only record we will have to trace your package. You will be required to provide this tracking number if your shoes are not received by the Mephisto Recraft Division.

9. **Very Important** Make sure that you fill in the Control Number (daytime phone number). This is the number you will use to make inquiries about your shoes.

Also remember to keep a copy of this form for your records.
